The process During the day. Daily. Sporadically.
During the day: Patients are asked to help us help them. They just need to log their symptoms directly into the system once in the morning and once in the evening. Daily: Telehealth volunteers contact home patients. They listen and connect, tracking basic symptoms and severity in the app. All details and trends are readily accessible by the patient’s doctor. If symptoms worsen, the doctor is contacted. Sporadically: Doctor and medical staff can see a live dashboard and focus only on patients that need more attention.
